• This is an interactive form created on Microsoft Excel that should be completed electronically before printing
• To be completed for each ALDI sponsored event

School Board Project Request Form

• Every year of the ALDI Initiative school boards are invited to submit a request form
• These school board projects will be posted on the ALDI website

Mentoring Project

• ALDI supports and mentors a developing colaborative school team
• Requests must be submitted by school board Director/Coordinator of Student Services
• First come first serve basis
